Occurrence report

Use this form to report anything safety related from operations at Nitra airfield. Report what happened — and also what nearly happened.

Reporting carries no penalty

The purpose of a report is to learn, not to find someone to blame. Nobody is penalised for reporting an occurrence they were involved in. This is the just culture principle. The only exceptions are wilful misconduct and gross negligence.

That is exactly why small things are worth reporting. A hazard can only be fixed once somebody knows about it.

Serious occurrences must also go to the authority

Accidents, serious incidents and occurrences subject to mandatory reporting must, in addition to this form, be reported to the civil aviation authority through the European portal aviationreporting.eu, as required by Regulation (EU) 376/2014.

This form is the aerodrome operator's internal reporting system and does NOT replace mandatory reporting. A report filed here anonymously does not discharge that obligation.
